Registration Questionnaire

Note: Attending at least two support groups or counselling sessions prior to joining the Six STEPS program will provide you the opportunity to help engage more effectively with the program and maximise its benefits.

Please review the questions below and consider your responses. If you have answered "no" to questions 1, 2, or 3, or are uncertain whether this program is suitable for you, we recommend speaking to one of our facilitators before proceeding with registration.

2. Are you ready to try something new by prioritising your own needs while maintaining connection to the person you support?*

3. Have you previously attended either one to one counselling or a support group in relation to this situation?*

4. Is there any additional information you would like to share with the organisers to help ensure your safety and well-being during the group? This may include considerations related to personal safety, such as unique living situations or any concerns you feel comfortable disclosing.
